That's at beginning, meaning when he killed that slugman, but since it's been a long time i'm not sure. It's clearly quite old, you can tell by the way it's drawn.

You are still active Reapers Ghost and i just looked at the manga due to the anime announcement which will adapt this very arc. (Unless the preview picture is a lie.)
Sorry BlackOrion but the often shared picture is from Chap. 98 - By Air and a lot of the slasher-psycho Guts imagery is from that very arc called Conviction.

He done show it when he gone poo poo on everyone in town. In one chapter an ox poo on a man until he dies from it and another man slips in his own pee. In the end of Berserk all the black guys show up to prove that Guts was right all along and a giant black man with a brain for an afro causes the end of the world. That is the story of Berserk.
