I just read Ichi the Killer. And i thought it was pretty good, bordering on outright immoral and ultraviolent themes. And i have read Berserk but it was mehhhh.

I am looking for something much more. Something that is totally brutal, no bullshit, contains rape or immoral sex with underage, murdering of children and adults, those NO-HOLDS BARRED stuffs you know...

Something that is not for the faint hearted and not a subject every author or audience want to touch.
Yes, I am looking for those sick levels.

I'm surprised you think Berserk is meh, the eclipse scene is so... Well, it's shocking to say the least. But oh well.

Indeed, Shamo's main character is, to quote the title of your post, seriously brutal, violent and with no morals whatsoever. He starts to change in the last chapters though, not like he stops being violent but he's not a sick bastard anymore. Which is good, because he was seriously disgusting.

Also, Mugen no Juunin character Shira may fit your tastes. In my opinion this manga is not so great and though it's generally gory, all in all the story itself is not what you seem to want. But Shira's scenes do fit your description. They fit very much.

I don't remember much, but you may want to check Sidooh out. I remember it touched with no care at all a lot of sensitive subjects (some very explicit rape scenes and other sexual stuff), so much that I stopped reading it as soon as my favourite character died... Which was pretty soon. So yeah, I'm not sure about this one but it wouldn't hurt to check it out.

I also remember Zetman having some really shocking scenes like that... At least for me. They're not recurrent and not in the beggining though, it was a particular story arc where one of the characters was impersonated by a sick bastard that basically raped a bunch of the character's fangirls. Or something like that... I don't really remember, hehe. It does have that mature and adult undertone though.

Basically I'm just going through the mangas that I've read that somehow left an impression of me because how crudely they treated subjects that I think should be handled with more care. But I'm kinda sensitive so I dunno, maybe you'll think this is all for babies~

Gantz8/10 is inconsistently dark but The World Is Mine7/10, Wolf Guy - Ookami no Monshou 6/10, Ookami no Kuchi: Wolfsmund9/10, Jisatsutou9,5/10, Battle Royale 5/10 and Jiraishin 7/10 are not. And google Avatar Press comics like Crossed to learn how dark thing can actually get ! Or mangas by crazypants MATSUMOTO Jiro or the gekiga style.

Would definitely second Shamo and anything by MATSUMOTO Jiro.

Anything by TAKAHASHI Tsutomu (sidooh, jiraishin) fits the quota but in my opinion all his works, though dark, feel very hopeful. (they're still good)

Anything else by the author of ichi the killer.

Manga´s by KITOH Mohiro will give you depression. The villians in Kiseijuu10/10 have no morals and the violence is hardcore. Eden - It's an Endless World!8/10 is about a world of hell. Satanister 6/10 and his other works are a fun pocalypse. Kirihito Sanka9,5/10 proved that Tezuka could do dark seinen like the best of them. Azumi 9/10 is shockingly nihilistic.
Black Joke6/10 has no morals. Terra Formars 7/10 is a fun kill em all. Gangsta. 7/10 has morals but drags now. Black Lagoon 8/10 can get nasty but fun but strated to drag too. Hellsing 9/10 has corpse rape ! Front Mission - Dog Life & Dog Style 7/10 is dark war book with almost no connection to the games. Heat 7/10 and his other stuff have manly Yakuza infighting.Innocent (SAKAMOTO Shinichi) 8/10 is just insane now. Ikigami 7,5/10 is dark a dystopia. Dorohedoro 9,5/10 is a lovestory with GORE. ASANO Inio knows psychological darkness. 17-sai. 8,5/10 is based on a real case. Seinen by KOIKE Kazuo like the untranslated Jikken Ningyou Dummy Oscar ?/10 can be streigh out insane ! How did he come up with this ? Finaly look at Go Nagai for some of the worst mangas of all time. I just hope you like "edgy" rape and canibalism to the MAX. Look at clips from Genocyber in the meantime because old ova´s were insane !
And now dear gorehounds look at my review for Psycho-Pass 2 and start watching it now(skip season 1). IG just gave us 90s ova insanity back and now even on tv ! So much "clean" murder fun.

Try out Garth Ennis's´s Punisher (Max) 9,5/10 now owned by Disney ! One of the arc´s(the Slavers) was about sexual slavery and baby murder. Wow ! Or DC Vertigo´s Unknown Soldier 10/10 about 90s Uganda. So WB put out a comic where child solders rape a nun on screen ! See america can be crazy too. Or read American Psycho(the movie is cool too). Let´s keep this master list going.

The already mentioned Yamikin Ushijima-kun is way better at live in the underound. And yes Berserk is THE best. True literature.

You didn't really say what genre so I'm gonna suggest Love prism (tohjoh asami). I'm sure the mangaka wanted to romantize the characters and what they do but it didn't work, short of a third character acting all preachy. It's just a hardcore yaoi with bdsm that comes off as torture more than anything. It's unfinished due to the mangaka's death but you can guess where they were going with the series.
